E Block Loading Dock

Please be advised that the E Block loading dock has recently been 
cleaned up and is not to be used for dumping items or as a place to hold 
equipment and materials for pick up.  All items are to be left in the 
lab or area of origin and a disposal form is to be filled out. 

The procedures for disposal and pick up are as follows: 

.    Go to My U of C
.    Enter your eID and Password
.    Click on Campus Services on the left hand side
.    Click on Materials Management
.    Click on Materials Management Service Request Forms
.    Click on the appropriate form (e.g. Equipment/Furniture Disposal 
Request Form)
.    Fill out then submit.
.    Print a copy and attach to the equipment/material
